  • Patient education on how to improve spoken, written, and nonverbal communication techniques

HOME HEALTH AIDES

TRANSITIONAL CARE SERVICES

  • Transitional Care ensures the coordination and continuity of care as patients transfer between different locations of care
  • The focus is on accurate relay of patient information, logistical arrangement
  • Patient/Family education and care coordination among clinicians involved in the transition.

CONGESTIVE HEART FAILURE MANAGEMENT

  • Designed to improve the quality of care and prevent the re-hospitalization of CHF patients
  • Our program will provide a comprehensive education on the proper management of CHF
